adding service eligibility map to referral page #273

Adds a referral eligibility map of all services to the referral overview page:

@esmithayer what do you think we should put inside the popups of each map marker? I was thinking any combination of address information, link to the service profile, and a deep # anchor link to the service information further down the page. Currently just service name and address:

Looks nice! What service info is already on this page? Like all the estimated prices etc? Address + link to that sounds good to me (and that has links to the full service profiles).
