"Patient" row in household member section is unclear #95

Going to: -Change "Relation" column label to "Relationship" as user suggested. -Describe the relationship as "Self" rather than "Patient" as user suggested. -Put "Patient" in the name column instead of "None" if patient hasn't entered a name yet. -Make DOB and SSN fields empty instead of "None" if patient hasn't entered those yet.
