"Request Closed" note should be shown at the end of the activities list #49

When the original SR and all its follow-ons have been closed, an additional dot should be shown indicating when the request was closed.

This is related to #47

A good example of this for testing is 12-00724508 (http://servicetracker.cityofchicago.org/requests/12-00724508). It demonstrates this issue and #47; the "closed" activity is in the middle and not at the end of the list where it should be.
