(we can track if this becomes problematic) Maybe at the beginning or end of the same page as the medical records request form, with the following blurb:

Vocatus is tracking the results of medical records requests to inform a national campaign for patient data access. Your contact information will only be used to follow up with you about the result of your request. The answers you provide in follow-up surveys by email or text may be anonymized, aggregated, and shared to educate the general public on the state of medical records access. We will not never share any personal or identifying information without receiving your permission.

Your healthcare organization may charge you for copies of patient records, whether in paper or digital form. This is especially common if you are making a large record request.

• I'm still tweaking the med records request form on the content and transport of records, I tried the current one by efax with my doctor's offices Friday and they were a bit confused.

If you decide to report your healthcare organization or provider to the Office of Civil Rights and they determine that it was a violation, OCR may fine and/or publicly identify your healthcare organization or provider.

If requested, OCR will keep your identity confidential during any investigation, and it is illegal for your healthcare organization or provider to retaliate due to the compliant. However there is still a chance that retaliation will occur, and/or your relationship with your healthcare organization or provider will be negatively impacted.

high importance (dare I say tomorrow @unclenate ?):

• put Vocatus at top instead of Unpatient - Done "ND"
• Remove: "These regulations have recently been amended to provide individuals with the right to receive copies of their health information directly from laboratories." - Done "ND"
• Remove: (because I think it will confuse people who are asking for records outside a specific procedure): "I had ( ) in my physicians office ( ) at the hospital ( ) on ( )." - Done "ND"
• Change "I Request Copies of the Following:" choices to those in the Google doc - Done "ND"
• Paper records: they could still scan and send these electronically. So instead suggest changing prompt to: "I prefer to receive my records on paper, please... etc" - Done "ND"
• Paper Records: you can't read the whole prompt. suggestion "Name of person/entity receiving records" and "Address or Fax" - Done "ND"
• See Google doc for MAJOR changes to Paper Records, Electronic Records, and Patient portal sections/options to reflect all the research I've done over past week on a coherent way to request records electronically. - Done "ND"
