If you decide to report your healthcare organization or provider to the Office of Civil Rights and they determine that it was a violation, OCR may fine and/or publicly identify your healthcare organization or provider.
If requested, OCR will keep your identity confidential during any investigation, and it is illegal for your healthcare organization or provider to retaliate due to the compliant. However there is still a chance that retaliation will occur, and/or your relationship with your healthcare organization or provider will be negatively impacted.

Your contact information will only be used to follow up with you about the result of your request. The answers you provide in follow-up surveys by email or text may be anonymized, aggregated, and shared to educate the general public on the state of medical records access. We will not never share any personal or identifying information without receiving your permission.
Your healthcare organization may charge you for copies of patient records, whether in paper or digital form. This is especially common if you are making a large record request.
