Add a donation link to the site.

The link should go to https://secure.codeforamerica.org/page/contribute/default?brigade=Code%20For%20New%20Hampshire

This is waiting on approval from the national Code for America organization but we can put the link in and have the branch waiting and ready for when we sort that out.
