add instructions to migrate external links migrations #3

These are needed in order for this plugin to function.

I think more or less linking to the pageflow-external-links installation instructions should be enough. There are additional requires steps like requiring/importing assets, but I'd rather not repeat the complete readme of the external links gem in here.
