Remove support for the (previously) optional COMMAND_PREFIX environment variable
Update README to include how to get application and server IDs for new (required) environment variables CLIENT_ID and GUILD_ID respectively

package-lock.json seems to have changed since I ran npm audit fix which fixed a severe vulnerability, while 4 moderate vulnerabilities remain. If that is undesirable, I can revert package-lock.json in the next commit.
