Bug: "Vaccination History" deselection crash #24

On the Covid vaccine page, deselecting an option for a vaccine in the Vaccination History causes an "Unexpected Application Error".

As part of the resolution to this, we are no longer allowing deselection of options in any of the fields, since none of them allow multiselect right now.
