Add "Can I waive Cardinal Care?" banner to top of waiver page

The banner should have the following functionality:

• Stay at the top of the page as you scroll down
• If not all questions have been answered, it should be gray and the the text "more info required" should be on the right side of the banner, as in the Figma
• If all questions have been answered, the logic program "waiver rules" from this page https://github.com/codexstanford/cardinal-care-dev/blob/main/claims_analysis/cardinal_care_waiver/cardinal_care_waiver.js should be queried for "can_waive".
  • If can_waive is true, then the banner should turn green and the "more info required" text should become "yes"
  • If can_waive is not true, then the banner should turn red and the "more info required" text should become "no"
