Description of the Change

Checked that --reconnect must be combined with --continuous. Prefixed all output lines with 'ACD Debugger: ' such that they would be more visible during itests. Fix readme duplication.

Verification Process

debug a security exception and ensure the output lines are all prefixed
attempt to use --reconnect without --continuous

Applicable Issues

Fixes: #31
