Project Card: Create 'Request to Join form' modal

User story: As a user, I would like to see a form on the page when requesting to join a project so no redirects are necessary.

Deliverable _

This ticket doesn't include the backend sending of the email, only the modal

_ Clicking on request to join will pop up a modal with a form that includes the following fields:

• Full name
• Email
• Github profile link
• Message - with label text 'Introduce yourself and include any interests'
• Send button (Doesn't have to send an email-yet)
