[#62] Filtering Part I (Modal + Styling + Behavior)

Summary

This PR adds the modal, filter form, new form fields, etc. It does not have tests yet because I'm trying to get this up so we can finalize functionality first. Additional styling will be necessary as well. This just roughs it out so we can implement filtering functionality.

Partially addresses #62 (issue)
