Reorganize, clarify document #3

The actual language is not modified, but the document was reorganized and formatted for clarification for ease of access. Some changes:

Added table of contents (with doctoc)
Separated goals into own section, moved to bottom
Enumerated headings
Split into sections: Framework and Goals

This was forked off of #2 for easier review. That PR can be closed if this one is merged directly.
