Download procedures from MDS #4

The current hard-coding of procedures in the Android client is not ideal. We 
should replace this by allowing the phones to download a set of procedures or 
forms from the MDS, which in turn could get them from the EMR backend.

The OpenMRS XForms module already supports enumerating and downloading forms 
available 
on the server, so there is no OpenMRS side to this task.

The basics are done now -- the phone downloads all available procedures from 
the MDS 
when you sync it. We could do more in this regard and let the user choose which 
forms 
to download but for now I think that detracts from the simplicity of the phone 
interface.
