Doctor Group selection on the phone #9

Allow selection of target doctor group on the phone. This involves either:

1. Fix the group in the procedure XML
2. Present a selectable list of user roles live/cached from OpenMRS

Once the phone is able to specify the appropriate group, then the queue 
module needs to be updated to only show certain queue items based on the 
given role of the logged in user.
