When an add on injects CSS and js in the page, how does the theme ensure that they can be included? What about inclusion of HTML generated by the add on? I am thinking Plone.app.toolbar here.

Good question. There is no automated way that I know of except to include everything in the head tag which I don't really want to do if I can help it.

You should really consider using the css registry and js registry. Makes it a breeze to support addons and stuff.
