May have to categorize code archive urls

Expose URLs in publication curator form so that code archive url can be categorized.

Plan:

[x] create CodeArchiveURL model
- url
- category
- status
- creator
- last_modified
- date_created
[x] relate CodeArchiveURL to URLStatusLog
[x] data migrate code_archive_url field on publication to CodeArchiveURL model
[x] add data entry form / popup on publication curator page to support add, modifying and deleting URLs
