Yearly update to Code of Conduct. Mostly minor changes.

In approximate order from important to clerical:

Dropped all mention of an external ombudsperson. We tried and tried to recruit someone, but failed.
Added note to encourage people to reach out to committee if they just have questions.
Made document consistent about when and how we report to the community: Whenever possible.
Updated board membership. Dave C is dropping Jaime RG is joining.
a few clarity edits
Dropped table of contents. GitHub now automatically generates this.
Removed trailing spaces. (I apologize for the noise.)
