Duplication of 'how to create a stream/table' docs

We've got duplication in our documentation that it would be good to get rid of, as there is a cost to keeping it updated.

"How to create a stream" is documented here:

"How to create a table" is documented here:

• https://github.com/confluentinc/ksql/blob/master/docs/developer-guide/create-a-table.md

And then https://github.com/confluentinc/ksql/blob/master/docs/tutorials/examples.md covers examples of create stream and tables.

It would be good to combine these!
