10 years ago update: figured out it was the un-documented requirement for "megajules"... (10 GJ in this case). Which begs the question of why I have to blow my entire parts budget on the pathetic little radiators available, more than 80 on the craft right now, and still not have enough power. =(
I put the vista drive in my "forward drive configuration.

To great effect. I was so thrilled that I made a new design called "The Grand Drive" with all tweakscales maxed, (not quite), and then put it on my most sophisticated 5M launcher.
Even with FAR, the rocket flies like a dream up to the circularization burn at 240k where it is finally time to drop the first stage and fire the vista where it promptly produces zero thrust, the second stage is 200 tons so you get an idea of how beefy my first stage is... There is no UI-visible indication of what the problem could be. I have tried setting up just that first stage on the pad and it will produce thrust for a split second, up to a few seconds, then go dead and just fall. Nothing accessible to the UI will make it work again. I'm afraid to touch the one already in orbit for fear of messing it up, I have already built a starship around it... I also tried different throttle settings ranging from just barely on to full power, no discernable pattern.
