adding conjure+ #33

We changed %%conjure to not be incremental. Incrementality is achieved through %%conjure+ and %%conjure resets the model by default.

%conjure_clear is also removed.

I'll release this as 0.0.8. If you use a previous version your notebooks should continue working of course.
