Updates jQuery to 2.0+
Removes oldie fallbacks and ie* classes on the html element
Removes respond.js and oldie.js
Removes application of nth-child classes to *-up classes and use nth-child selectors in CSS
Cleans up icon placeholder usage

Closes #83.
