Describe the bug Not seeing the Accept changes / discard changes button in diff review

To Reproduce Steps to reproduce the behavior:

  1. /edit improve code

Expected behavior I should see the Accept changes / discard changes button in diff review.

A couple questions so I can narrow this down: Is this sporadically happening, or consistently? And does any amount of a) waiting, b) scrolling, or c) changing tabs cause them to show up?

Consistently. No to A), b) and c)
