Language code popup panel was not initialized correctly

What steps will reproduce the problem?
1. go to the language config panel
2. choose add
3. choose "Not in list"
4. do nothing and just close the popup panel
5. rechoose add then open the popup panel again

What is the expected output? What do you see instead?

The popup panel was not initialized correctly, "Not in list" was checked but 
the default drop down list was shown instead of the input box of customer code.
