Move existing documentation to Sphinx #20

Move from Mkdocs to Sphinx.

Ultimately I decided not to move to Sphinx, but to stay with MkDocs and move from Read the Docs to GitHub pages. Sphinx just wound up being more complicated and less readable than I'd expected (even taking into account the fact that they support Markdown).
