fix: clean up documentation and configuration #192

The distinction between server TLS and proxy TLS configuration wasn't clear and partially wrong. There were also a few discrepancies in variable defaults and naming.

use separate environment variables for configuring server TLS / proxy TLS
update documentation on environment variables
align variable names to reduce the number of distinct env vars
align default values of env vars (e.g., enable OCSP stapling for all images)

Fixes #166
