Switch high and low volume nibble. #36

Set instrument volumes in high nibble and track/cmd volumes in low nibble. This should reduce shifts in replayer code.

Although this is a CPU improvement. This is a lot of work and gain is relatively small (maybe 128 cycles per ISR)
