Staffing estimation model #93

Work started in this notebook to estimate staffing needs based on this study: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC4636910/

The goal of this issue is to complete and test this staffing model, and integrate into covidcaremap package so that it is easy for analytic notebooks to pull this data in dynamically for different regions and scenarios.

I have noticed across institutions, beds and ventilators are now very well covered - however, nothing on staffing ratios yet!
