Radio buttons on Form A are not working as expected

Issue description

Radio buttons on Form A are misbehaving. Different Radio Button groups are acting like they are on the same group. For example if I select any gender in Form A - Part 2, and then proceed to select another radio button on Form A - Part 3, the gender radio is dis-selected.

Expected behavior: Unrelated groups of radio buttons should be independent. Selecting a Radio Button on Part 1 should not dis-select a Radio Button on Part 2.

Steps to reproduce:

• Head over to 'Form A' page.
• Select any gender from Form A - Part 2.
• Select any radio button on Form A - Part 3.
• Observe gender radio button get reset
