CS016 Address 39 specific comments in edited DIS document

An edited file of the DIS has been prepared by ISO/CS. Please note that the DIS edited file contains 39 comments on specific elements of the document, which need to be fixed so the document complies with the Directives, Part 2.

Please take all the comments in to consideration to avoid rejection at the next stage.

I've made a pass through these, and I think #393 and #395 capture the only remaining open issues here.

• hanging paragraphs/single-sublevel: this is #394, Accepted.
• Modal verbs: this is #393, Accepted.
• Numbered notes and examples: this is #398, implemented by cplusplus/draft#4190.
• Acknowledgements/Bibliography/Trademarks: implemented by cplusplus/draft#4175.
• Remove "normative": implemented as part of cplusplus/draft#4168.
• Remove "normative references": implemented as part of cplusplus/draft#4214.
• Change "This Clause" to "This Annex": implemeted by "[depr] Don't claim this Annex is a Clause.".
• Cite Annex D as normative: implemented by cplusplus/draft#4189.
• change "This Annex" in Cross references: implemented by "[xrefs] Don't claim this appendix is an annex."

This concludes the review. All PDF comments have been address, and all but one have been accepted. The one that we rejected is a reference to CS-19, #395.
