Cave improvements and lives for second boss level #253

right now the doorway is always there from the start of the level

i think that was once i had the cave rise up, and then went back to the main menu and restarted the level.

other than that bug it looks great

@averybrown that's cause I forgot to destroy the entity. I will push a fix just give me a minute
