The links at https://github.com/DBCG/cqf-ruler/wiki/Quality-Measure-Evaluation-Walkthrough either return "unknown" response from the FHIR server or 404 errors.

The connectathon repo is no longer updated. Need to migrate the documentation to reference ecqm-content-r4 and ecqm-content-r4-2021 repositories.
