Opioid IG: add CDS Hooks reference documentation to all of the CQL and IG #240

As an implementer of the CDC Opioid guidelines, I need an implementation guide with better documentation of the CDS Hooks framework and how it's used.

Based on feedback from MITRE (see MIH-47 or attached PDF on page 9): 

The Opioid recommendations follow some conventions, particularly around the use of the expressions “Get Summary”, “Get Indicator”, and “Get Detail”. My understanding is that these correspond to the similar properties on CDS Hooks cards. As far as I can tell, however, this is not clearly documented in this IG. I think implementors need to have a better understanding of how these special expressions are intended to work – and that should be documented in the CQL (to some degree) and in the IG (perhaps in more detail).

Acceptance Criteria

  1. CQL and IG files have been updated to reference CDS Hooks in comments
  2. All changes have been committed to the repo (https://github.com/cqframework/opioid-cds), pull request created, approved and merged
  3. Opioid IG builds correctly

Ideally these expressions are removed from the CQL where possible. We should be attempting to follow the mappings provided in the FHIR Clinical Reasoning Module here: http://hl7.org/fhir/clinicalreasoning-cds-on-fhir.html#evaluation. Then we should point to that documentation when discussing how the PlanDefinition resources are structured to support CDS Hooks card mapping
