updated naming conventions

there is a separate ticket to update the links, for the draft replacements the links and the naming have been updated at the same time. For more information on draft updates, see last commit here: https://github.com/cqframework/opioid-cds-r4/pull/315

Checking to see if the IG name is updated per Chris's comment - IG name: "Clinical Practice Guideline for Prescribing Opioids Implementation Guide". This should be the name that appears on the 2022 IG. Thanks
