updated the outdated information, old links and content to 2022 content

please note that:

the current response to the hook request does not contain any suggestion element the CDS sandbox does return an empty list of cards.

@alackerbauer This PR still needs the changes requested in the 2016 PR (#337)

All changes from the 2016 version have been applied, however, this error still occurs in the sandbox because the patient "patient-example-rec-10-patient-view-no-screenings" does not have a name element. Should we add one?

Broken Links in 2016 Version

If the genonce script is run locally the links work without changing anything and this error does not occur. In the published version, however, it only works if a capital letter P is used in the filename: Patient-example-rec-10-patient-view-no-screenings.json For the medication Request resources it works locally as well but for the published version the link has to be changed to MedicationRequest-example-rec-10-patient-view-no-screenings-context.json

These changes also work in the local version. PR #346
