Indicate whether opioid MME calculation contains more than just Table 1 ingredients #3

The calculator needs to be clear about when the opioid under consideration uses conversion factors from the MME table here: https://www.cdc.gov/mmwr/volumes/65/rr/rr6501e1.htm#T2_down

Based on discussions with the sites, this will be implemented as a configuration point. So there will be two conversion tables in the default, one for the Table 1 content from the Guideline, one for the research content from the MME data file. Then the calculator needs to be able to be configured with the conversion factors. The conversion factors also need to be able to indicate that a particular ingredient is known to be Opioid but has no conversion factor and so the resulting MME needs to come back with a flag that the calculation is incomplete.
