Split theme and plugin into separate repos

We need to split the current commoners repo into two

• repo for the commoners WordPress theme
• repo for cc-global-network plugin

Bare repositories created:

• creativecommons/wp-theme-cc-commoners
• creativecommons/wp-plugin-cc-global-network
