material-dashboard-pro HTML v2.1.2 responsive bug #285

Open the page material-dashboard-pro-html-v2.1.2/examples/forms/extended.html

Resize it a bit and you'll see something like the attached image.

How can you fix this?

I need this 2 times: to have a "type of phone" + a text field "phone", and a "type of address" + a text field "address". And as you might guess there are many times where you need a select option + an input next to it...
