the fix consists of

• removal of "index" parameter
• resolve to "index.html" if url basename is empty or url extname is empty.
• on "exclude" paths, call res.writeStatus('404');, res.write('404 Not Found');, and res.end(); - instead of calling req.setYield(true).
• attaching of res.onAbort on all requests (encountered a throw on GET requests)
