Doctor can make appointment with himself #2

perhaps make list of NRIC used so that no duplicate can be made or exception for when the same NRIC is used in add appointment command

The app is meant for administrators to manually add doctors and patients. There might be a case where the person is once a patient, but now has become a doctor at the same clinic.
