Fix issue https://github.com/criteo/cassandra_exporter/issues/49 Little fix visualization at dashboard "Max Last GC duration", add node/server names to legend. For more clarity.

Hello, First, Thanks for the PR :) I am in vacation for several weeks, when I return I will check if it still works with grafana 5 and if yes merge it.
