What is best practice to record contact hierarchy...

This is a really good one! Interesting - we have a similar structure with Users and Managers/Supervisor but having this on Contact could certainly be useful.

I know we can consider using Connections instead due to their reciprocal nature, but i wonder if that would really solve or help what your looking for, as this isn't the way it is managed in the User entity

Connections are a bit awkward for most users to work with, and they don't work in the OotB hierarchy view, which you might hope you could use for simple relationships (although not the peer-to-peer one my Team example implies)
