Support for some markdown basics in PDF renderer #418

This might be overkill / too tricky to get 100% correct. But maybe we can support some simple things like bold and italics.

The other nice-to-have about this would be if we plug into unified for the markdown parsing we can automatically not render things like the :no-refs[...] directive syntax in pdfs
