It will be hard to tell you how to reproduce, but I guess it is somehow when you play with tasks management, killing task or reordering them:
In this screenshot you see that the w1 and w3 workers are displaying the same task. They are perfectly in sync in terms of progression. The only difference is the heartbeat date.
