Add PR and ISSUE templates

Description

This is an idea for implementing Github templates to help with bug reporting and collaboration by the community.

Motivation and Context

While not having templates and guidelines is ok, I believe that adding templates with the minimal information required to speed up bugfixes and oriented discussion of features and enhancements is a great quality-of-life improvement for those that use the application and will use it in the future.

Types of changes

• [ ] Bug fix (non-breaking change which fixes an issue)
• [ ] New feature (non-breaking change which adds functionality)
• [ ] Breaking change (fix or feature that would cause existing functionality to change)
• [x] Documentation changes (Change on guidelines, updates or instructions)

Checklist:

• [ ] My code follows the code style of this project.
• [ ] My change requires a change to the documentation.
• [ ] I have updated the documentation accordingly.

This is just and idea, since I'm a user of this project. If you think this is pertinent, some changes on the README need to be made. Let me know what you think and any changes needed.

I tried to implement the same structure found in: https://help.github.com/en/articles/creating-issue-templates-for-your-repository but I am not sure if this will work if not using the interface to create.
