We need a simple questionnaire to capture basic information about the organizations that want to participate in this summer's EDEN Pilot project.

I created a copy of the questionnaire in the Wiki: https://github.com/cryptotechguru/EDENomicon/wiki/EDEN-Pilot-Questionnaire

We can co-edit the questionnaire in the Wiki. Once agreed on a version we will post a PR to update the copy in the EDENomicon Projects folder.
