General overhaul of the templates for issues and PRs

This is useful, because the creator now has to make a conscious decision to either delete the "chapters" of the PR and is thus reminded of them, same goes for issues, where now the creator has to consciously ignore or delete the chapter. Hopefully this template update will lead to higher quality communication of desired changes / changes, and thus higher productivity and less questions about issues.
