Create prize receipt forms #483

Need forms for users to fill out when they win a prize. This will require the SSN to be collected to be passed to UH fiscal folks. Hopefully the form can be machine generated. See issue #396

fixed. now the system support custom template for "prize-winner-receipt" under "internal admin/Notice templates", to be used in different organization. for example, HPU might not need the SSN.
