Add generic missing identification #5

Some systems do not record the exact kind of identification that is missing from an FPCA application. ApplicationRequestStatusType only contains missing-ssn and missing-state-id-number. I propose that we add a third, generic value, named missing-identification, which will serve as a catchall.

Not to be too pedantic, but missing-identification or missing-identifier?

In VRI we use the term identification, so I'd prefer that.
