Counts and rates are both done. Need additional testing to determine why we can't remove dynamicMonthStart without breaking the visualizations (refer to issue #20).

Background Two types of measures are currently displayed in this measure: Point-in-time (the information in the bar charts) and flow or cohort measures (the information in the line charts). We want to change this set of measures such that the point-in-time measure (i.e. the bar charts) no longer displays. For the time being, we want to preserve the underlying stored procedure and make this change entirely through our config files. Specific Requirements
